I think the standard medium for instruction around the world is English. I become curious with your question now, honestly. I guess if you really want to study in there, you'll have to inquire at any university you'd like to be in because they'll be the ones to inform you about this.

I can picture American universities offering a limited curriculum to US college students seeking to study abroad for 1 term or for a year.
But, I have difficulty seeing a reputable Italian institution offering a doctorate with classes all in English. Most European university students have a reasonable facility with multiple languages. I would expect some of the readings to be in English, Italian, French, German, etc.--just as they might be here in the US.
Perhaps times have changed. In the old days, many departments required PhD students to high pass specific language exams. If you didn't know French or German or whatever, you were expected to pick it up on the side.
So, I say turn this into a culturally expanding experience and invest a year seriously engaged in an intensive Italian program someplace in Italy. Buona fortuna!

I cant really gove you an answer,but what I can give you is a way to a solution, that is you have to find the anglde that you relate to or peaks your interest.
