As long as there is sufficient oxygen the reaction goes to completion and all you get is water and CO2, but when the combustion is slightly inefficient due to insufficient oxygen, CO is generated instead. In a car engine there is less than perfect combustion efficiency, so a slight amount of CO comes out in the exhaust, along with water and CO2. If you have a charcoal fire in a closed room, as the oxygen gets used up in the burning coal, generation of CO starts, and people have died from being in such situations.
