Have any states revised their voter registration forms after HAVA? Has the EAC revised the national mail form after HAVA?

Section 303 of HAVA requires many States to undertake changes in their voter registration forms. The revised national mail voter registration form is available on the Election Assistance Commission website. The Department has also received a number of requests from States for examples of how other States have revised their forms and has obtained examples of registration forms from two States - one from a State covered by the NVRA, North Carolina (PDF)* and the other is from a State exempt from the NVRA, Wisconsin (PDF)*.
