How can I disable and enable form using checkbox?

You can always use a java script to enable or disable controls in html. I suggest you use the getElementById, or you can use this script I created: Put the script between head tags. I cant really gove you an answer,but what I can give you is a way to a solution, that is you have to find the anglde that you relate to or peaks your interest.
