Conceptually, the dual-plane breast augmentation is a complex procedure. Basically, the implant is placed above the muscle in the bottom third of the chest. The muscle is then cut widely and the implant is placed behind the muscle in the upper portion of the pocket.
The muscle is not repaired, allowing the implant to fall through it, held by breast tissue and skin. The implant position is more natural, with less upper bulging. It also does not disrupt the blood supply to the nipple, allowing the patient to undergo a lift in the future.
Finally, wrinkling is lessened, since the upper portion of the implant is behind the muscle. Since the dual-plane procedure was introduced, I have switched from 80% of my patients having the implant placed in front of the muscle to 80% receiving the dual-plane procedure.
