I would make an anonymous tip, I think. Not only is that person disgracing the competition, they are endangering their health. Since I probably couldn't convince them not to do it, I think them getting caught would force them to face up to it.
They don't deserve to win, or even compete, if they aren't going to compete fairly. Hopefully having that happen to them would make them feel like they had hit their rock bottom, and would help them turn things around. Steroids are dangerous.
I would try to be there for them as they faced the disappointment and embarrassment, but my support would be for them, not for their actions.
