You can request anything, no guarantee the doctor will find it "necessary" for you.
You will not get a card/certificate without living in that state. Some states are a little more strict, sometimes your illness has to be fairly serious, and some times they require an extensive medical history with whatever illness you need it for. California, however, it's easy to obtain.
