Maybe it's just her hair shedding. Having twists in the hair will not allow the naturally shedded hair to shed out. So when it's time to take the twist down, there will be three months worth of shed hair that needs to come out.
Hair sheds on average about 50 to 100 hairs a day so imagine what 3 months will leave you with. If her hair indeed was falling out maybe the twists were too tight.
