If you are reproductive (approx 10-50 yrs) and not on the contraceptive pill, then you will form cysts on at least one of your ovaries each menstrual cycle. These are called functional cysts because they are part of your ovarian function.

These can be up to 7 cm in size. If there is concern but nothing needs to be done urgently, say because of pain, then recheck the ovaries 6 weeks later with another ultrasound. If the cyst(s) were functional they will be gone.

If they are still present then they are pathological cysts and should be removed. If you are non-reproductive (pre-pubescent or post-menopausal, or on the pill) then your ovaries have no function and you can't have functional cysts. If you do have cysts then they are pathological and should be removed.

All ovarian cysts in a postmenopausal woman should be treated as malignant until proven otherwise and the treatment should be removal of the entire ovary and ideally the other ovary.
