To my recollection trained technicians Emergency staff should be applying casts. Like 'Serial' mentioned we do not apply a cast to twisted /sprained ankles. As far as the lewd gesture....that will be your decision.

I cant really gove you an answer,but what I can give you is a way to a solution, that is you have to find the anglde that you relate to or peaks your interest.
