I think that it might be due to the smoking and bad habits of his mother but it might also be due to the fact that people often forget things which they don't give any importance to. So, you can check whether that kid really cares for the things around him or not.
I'm no expert but I would think that if his memory loss is caused from his mother taking drugs and drinking, that the symptoms would have shown up much sooner.
I would suggest keeping a journal of what type of things he is forgetting. Is he forgetting things like where he put his shoes? (normal for a kid) or things like - something you have asked him to do?
I honestly do not think he is having memory loss but instead he is growing up and getting busy with life and busy with his own thoughts!
While there is a very good chance his mother's habits damaged his brain, there are ways you can make things better.
Firstly, have him practice on focusing. Daily. Not like Jedi training or anything, but maybe get a course that can help him develop his focus and critical thinking abilities.
The earlier you can get him started the better off he will be.

I cant really gove you an answer,but what I can give you is a way to a solution, that is you have to find the anglde that you relate to or peaks your interest.
