In the overwhelming majority of cases the injury is faked and so crutches or any other splint, bandage, etc. would not be used off set. For example, Dr. House does not actually have a limp when you see interviews with him. (He also has a British accent but that is another story.

) In rare cases, an actor might get injured accidentally on set and so truly require crutches. In this case the producers/directors/writers might have the actor pretend to get hurt in a scene just to allow them to use the crutches that they legitimately need.
