If you are talking about manufacturer coupons, they are created in cooperation with the manufacturer and the manufacturer pays for the site to put them up. I am not up to speed on who the companies and the processes are on the internet, my information is a little old, but I can tell you about the general process from when the only coupons were in the Sunday Supplements. The manufacturer decides they want to put out a coupon and they talk to the different companies that distribute coupons about their costs and the reach (how many people they can guarantee will see the coupon).
They they contract with the companies they choose, who then take the art work created and distribute it. Some of the same companies have websites that do sunday supplements (RedPlum, which is Vlassis and Smart Source) and then Catalina (the people who make the machines that print coupons at the registers) have another. Some companies have their own websites with coupons (Bettycrocker, pillsbury which are both General Mills, Campbells) and some just put their coupons on their general website.
While there are a few coupon aggregaters out there, most companies have sales people on the ground the actually create the coupon/deal. For example, a salesperson may call a restaurant to negotiate a daily deal, and they come to an agreement, say 40% off.
Coupon sites are partners to these companies. They have certain kind of arrangement to get more sales, that is offering coupons to customers. How they earn and divide profit depends all in their arrangement.
