Food Stamps is a federal program, so all you have to do is change your address. Food stamps are transferrable to any of the 50 United States, and maybe Puerto Rico, but I'm not sure about that. As for ADC, welfare programs are state programs that get part of their money from federal funding, so you would have to qualify in the state where you are now living.
AFDC payments and WIC are easily transferred, because they are funded 100% by federal funds, but housing and medical are not. For example, in Florida, you must have dependent children to qualify for Medicaid, but in some states, single people can be covered.
