I've seen this situation before in New York First of all, be honest with yourself. DID you really not know the woman was working through her lunches? Was there NEVER a time you saw her work during lunch?
If the answer is that you DID see her work through lunch then it was up to YOU to challenge her about it. It was up to YOU to ask her why she didn't go to lunch and if you needed her to stay you make sure her time sheet reflects this and she's paid. If you did NOT need her to stay you tell her to go to lunch and make sure she goes.
Having said that, if you really did not know she was working through lunch, if you did not NEED her to work through lunch, then you tell her an employee can not take it upon herself to work overtime and expect to be paid. Overtime has to be sanctioned by the Supervisor. Your Employee Manual will back you up in the section on "Overtime".
I'm sure that somewhere in that section it talks of how the SUPERVISOR would request that the employee do overtime. Since you did not request that she work through lunch, she can't get overtime. You also tell her that she was expected to get her work done without working through her lunchbreak and if she could not get it done, then why not?
You also need a meeting with your staff. At that meeting you make it clear to all that an employee can not take it upon themselves to work through lunch or work late or come in on a Saturday to work and expect to be paid. It has to be sanctioned beforehand by YOU, the boss.

I do not know Arizona Law, but... I would say she is SOL. If she did not think enough about the work to punch her own time card, she does not deserve the pay. You do that for one, you will have every Tom, Dick and Harry coming to you saying they worked overtime and forgot to punch the time card.

I cant really gove you an answer,but what I can give you is a way to a solution, that is you have to find the anglde that you relate to or peaks your interest.
