The most effective insults are ones that pull from the other person's personality/appearance/mannerisms, etc. Try and think of specifics you can use against her. For example, if she has blue hair, he can call her "Smurf". If she wears a lot of makeup, go with something like "Over-painted Clown".

If she's short, go with something like "Midget". If she's dating the richest boy in school, call her "Goldigger". If she wants to be popular, call her "Wannabe" or "Has-been".

If she's sensitive about her weight, call her "Tubby" or "Chunk". If she is not so smart, call her "Einstein" or "Brainless Wonder". Some generic ones: Freak Weirdo Psycho Lunatic Loon Pain in the neck Jerk Waste of space/oxygen Butthead Gasbag Loser.

Have the insult match her theme. If she's a cat, you can call her 'Whiskers', 'Fluffy', or 'Pussycat' Or, if she's like a Puppeteer/Puppet, 'Dollface' and stuff like that.
