Have him checked for a UTI but you also need to have his blood glucose checked again as well --- cats are notorious for becoming unregulated even if they have been on treatment for a long period of time ---at 14 you should also check his BUN/ creatine to check kidney function --- a routine chemistry panel will give you all this information .

After reading your other posts another thought -- if he has arthritis and it is not being treated he may be having difficulty getting comfortable in his box -- I have a 21 year old cat and for him I have gone to a a large tupperware underbed storage container and I cut down a small part of one side so he can walk straight in --- you might try something like this or medication for his arthritis -- but I would still make sure all his blood work and urinalysis is normal .

The litter box had a top layer (it is not a closed box) that snaps on the base to help keep the litter inside. Since he no longer covers up afterwards anyway, I removed the layer to make it easier for him to get in and out. He still pees around the edges.

While he is arthritic, he can, if he feels like it, make it up stairs. For a while I moved his litter box out of the laundry room to the garage and just left the door open. He would go out to the box in the garage which is down (and up) three steps.

He still missed the center of the box. So he knows to use a litter box. He does use the litter box, he is just not good at it.

His diabetes has gone into remission and then come back over the years. He is due for another curve shortly. I guess we will do a UTI test(?), some blood work and maybe have his semi gimpy leg/hip x-rayed.

Thank you. No check yet. I saw that as the possible explanation, but wanted to check other possibilities.

He does make it to littler box and ONLY the peeing is the issue. In his case, he is peeing on just about the 'inside' edge of the box. This seemed a bit different than the UTI reports like he wasn't even going in the box.

Neither he nor I would now be considered the sharpest knives in the drawer and we both creak along with arthritis, but we both still watch the political looney mill on TV and laugh and cry about it. On the dementia idea, one thing I did notice, he seems to 'rub his cheek' against doors, boxes, floor covers, etc more than he did before. And in the morning when I wake up, he will join me in the bathroom and seem to purr louder than I remember. Minor points that might mean something?

I cant really gove you an answer,but what I can give you is a way to a solution, that is you have to find the anglde that you relate to or peaks your interest.
