How is eligibility for Homebound Program services determined?

CMS student may be eligible for Homebound instruction if a doctor decides that due to illness, accident, surgery, etc. the student will not be able to return to the classroom for a period of 4 or more consecutive weeks of school. Eligibility begins the first day the student cannot attend. A student may be referred by a parent, school staff member or physician.

The referral consists of a doctor’s statement indicating the diagnosis and how long the student is expected to be unable to attend school signed by the physician submitted to the Related Services Program Specialist. If the student is certified under Exceptional Children Program guidelines the IEP team must convene to discuss and document, if necessary, the change in placement. More.
