If there is excess skin to be redraped, surgery is the best option. Thermage as discussed can have mild to moderate improvement; however, you can never tell who is going to respond well. I have not been that impressed with Thermage's results.

I often do Botox for tight platysmal bands. It works great! The Fraxel laser is in its infancy, but I have been pretty impressed with the horizontal neck line response.
