How does the system protect good teachers from being mistakenly or unfairly required to enter remediation?

Teachers are well protected under the OPE system. The first thing the Peer Assistance and Review (PAR) panel considers is whether the evaluation has registered, to use a medical analogy, a "false positive." In other words, they check for anomalies or consider extenuating circumstances.
