The engagement ring really depends on the couple. For some, the man chooses the ring without any input from the woman. For others, the woman picks out the exact ring she wants and tells the guy.
I'd say, based on the people I know personally, most couples end up somewhere in the middle - they look at some options together, the woman suggests a few preferences, and the man makes the final decision. For the wedding rings, the woman typically chooses her own and the man typically chooses his own. They don't have to match, and it accommodates their different styles/preferences for something they'll be wearing every day, forever.
Some couples choose the other's ring, but I don't get the impression that it's very common. Most people want a say in their own wedding ring. Sometimes engagement rings come in a set (either with a matching wedding ring for the woman or a matching set of wedding bands for the woman and the man), but they don't have to purchased that way.
