If you try to do it quickly you'll not only hurt yourself (engorgement is no fun) but you will shock your baby's digestive system and end up with a gassy and likely constipated and unhappy baby. So start slow. Start by pumping breast milk and weaning your baby from breast feeding itself.
This gets your baby used to how to bottle feed|bottle feeding. Always hold your baby while you bottle feed. Try to make the bottle experience as close to breast feeding as possible.
Once your baby has adjusted to the bottle slowly begin mixing formula into the bottles. Overtime decrease the breast milk and increase the formula. Remember only to pump as much breast milk as you need.
Eventually, you will no longer be mixing breast milk in at all. Another method is to replace one breast feeding a week with a bottle feeding. I however had better luck with the above method.

I totally agree with unwirklich and ai-ai! Slowly weaning off the baby as you and the baby are comfortable is really the best. At around 18 month a baby cuts its front teeth and that is the natural time for weaning.
If you have to or want to wean before or after, that is entirely up to you! I also just want to add a milk substitute that doesn't contain cow's milk or packet milk, it's plant based and very, very similar to mother's milk: You need a blender, water, peeled almonds and dried apricots. Blend those ingredients and the quantities vary whether you want to strain out a "milk" or whether you want to spoon feed a sort of smoothie to your baby.
Almonds and apricots have all the vitamins and minerals needed for an infant! And in The School of Natural Healing by Dr. John R. Christopher (amazon.com/School-Natural-Healing-John-C...), I found a passage in there I'd like to share with you.
Quote: "According to historians one of the greatest days in anyone's life during biblical times was the "day of weaning. " On that day, the one being honored was the individual that was once a baby, and now that it had cut its eye and stomach teeth was now a "child"! Family, friends and neighbors were invited to the weaning feast, and who sat at the head of the table?
The new child. This is in many instances the only time the individual ever had a special feast of its own with lots of guests, and it was allowed to be honored by sitting at the head of the feasting table. " Enjoy!
