How does the wellness incentive plan work?

There are several different incentives available through the employee wellness program. They are listed and explained below: HRA Incentive: All employees who work at least 400 hr/year and complete a HRA will receive a $20.00 credit on a Wellness Advantage Debit card, be eligible for 50% on KeepWell classes and 25% discount on Healthy Alternative therapies. Employees who have medical insurance with CMH will receive a reduction on their biweekly premiums and will be in the middle tier/rate.

Employees who do not have health risk factors associated with high cholesterol, high blood pressure, high body composition or smoking, will be eligible for an additional reduction on their biweekly premiums and will be in the lowest tier/rate. Improved Health Risk Factor Incentive: An employee must complete a HRA to qualify for this program. $25 per quarter will be given to employees who improve their health risk factors by making significant progress in moving toward a “desirable” status in one or ...
