I do think your advice is prudent, I should start with the 20mg, this will as you point out allow two things to transpire, 1) see if there are any of the typically rare side effects concurrent with a 20mg dose 2) get my triglycerides below 100%+ out range, I know my cardiologist feels that "time is of the essence" so this full scale approach is warranted, my PCP would take a more conservative approach so once again its a matter of OPINION, I am however prone to listen to the reason you have laid out before me.
