My cat consistently poops about 12-18 inches outside the little box, but urinates inside the box. How can I change this?

My cat consistently poops about 12-18 inches outside the little box, but urinates inside the box. How can I change this? My two cats are 11 years old.

They are sisters. They have always been together. About 6 years ago, one of them started pooping outside the litter box.

She does not urinate outside the box, only poops. And she does not poop in the box ever. It doesn't matter if the boxes (we have two) are clean or not.

We tried having 3 boxes for a while but it did not change the behavior. We have talked to our vet and she doesn't have any ideas. We have tried keeping the boxes immaculately clean, putting in brand new boxes every month, different litter types and brands, add additional boxes, moving the location of the boxes, putting boxes multiple places throughout the house, ...Suggestions?

I cant really gove you an answer,but what I can give you is a way to a solution, that is you have to find the anglde that you relate to or peaks your interest.
