Why did some recipients meeting invites appear in Outlook as an email only and without an option to Accept the invite?

They're probably using a Gmail account on Outlook. Google has recently changed the default setting for invites to be automatically added to your calendar instead of giving you the option to accept the invite. If you turn off the setting then the invites should show up in Gmail again.
